Clarithromycin

 Clarithromycin is a macrolide antibiotic used to treat a wide range of bacterial infections. It works by stopping bacteria from making the proteins they need to grow and multiply. As a result, the bacteria cannot survive, allowing the body's immune system to eliminate the infection. Clarithromycin is effective only against bacterial infections and does not work against viral illnesses such as the common cold or influenza.


Clarithromycin is commonly prescribed for infections of the respiratory tract, including pneumonia, bronchitis, sinusitis, and throat infections such as pharyngitis and tonsillitis. It is also used to treat ear infections, skin and soft tissue infections, and certain infections caused by Helicobacter pylori, a bacterium associated with stomach ulcers. In cases of H. pylori infection, clarithromycin is used as part of a combination therapy with other antibiotics and acid-reducing medicines to increase the chance of successful eradication.


The medicine is available in several forms, including tablets, extended-release tablets, oral suspension (liquid), and intravenous injections for hospital use. The dose depends on the type and severity of the infection, the patient's age, kidney function, and other medical conditions. Adults are commonly prescribed 250 mg or 500 mg twice daily, although the exact dosage should always follow the doctor's instructions. Extended-release tablets are usually taken once daily with food.


It is important to take clarithromycin exactly as prescribed and to complete the entire course of treatment, even if symptoms improve after a few days. Stopping the medicine too early can allow bacteria to survive, increasing the risk that the infection will return and contribute to antibiotic resistance, making future infections harder to treat.


Most people tolerate clarithromycin well, but some may experience side effects. The most common include nausea, vomiting, diarrhea, stomach pain, indigestion, headache, and an unusual metallic or bitter taste in the mouth. These side effects are usually mild and resolve after treatment ends. Taking the medicine with food may help reduce stomach discomfort, although this depends on the specific formulation.

These include severe allergic reactions, swelling of the face or throat, difficulty breathing, severe skin rashes, persistent or bloody diarrhea, yellowing of the skin or eyes (which may indicate liver problems), severe abdominal pain, hearing changes, or an irregular heartbeat. Clarithromycin can, in rare cases, prolong the QT interval on an electrocardiogram (ECG), increasing the risk of potentially dangerous heart rhythm disturbances. This risk is greater in people with existing heart disease, low potassium or magnesium levels, or those taking other medicines that also prolong the QT interval.


Clarithromycin interacts with many medicines because it strongly affects a liver enzyme called CYP3A4, which is responsible for breaking down numerous drugs. This means it can increase the levels of certain medications in the body, potentially leading to serious side effects. Medicines that may interact include statins (such as simvastatin and lovastatin), warfarin, digoxin, colchicine, some antiarrhythmic medicines, certain anti-seizure medications, and various sedatives. Because of these interactions, patients should tell their healthcare provider about all prescription medicines, over-the-counter products, vitamins, and herbal supplements they are taking before starting clarithromycin.


People with kidney disease or liver disease may require dose adjustments or closer monitoring while taking clarithromycin. The medicine should be used cautiously in individuals with a history of heart rhythm disorders, especially long QT syndrome. Older adults may also be more susceptible to certain side effects and drug interactions.


Regarding pregnancy and breastfeeding, clarithromycin should only be used if the expected benefits outweigh the potential risks. If a patient is pregnant, planning to become pregnant, or breastfeeding, they should discuss the situation with their doctor before starting treatment.


Clarithromycin is also an important component of treatment regimens for Helicobacter pylori infection. In these regimens, it is combined with another antibiotic, such as amoxicillin or metronidazole, together with a proton pump inhibitor like omeprazole. This combination helps eliminate the bacteria, allowing stomach ulcers to heal and reducing the chance of recurrence.


To use clarithromycin safely, patients should follow a few simple precautions. The medicine should be taken at the same time each day to maintain steady levels in the body. Missed doses should be taken as soon as remembered unless it is almost time for the next scheduled dose. Patients should not double the dose to make up for a missed one. Oral suspension should be shaken well before each use and measured with a proper dosing device rather than a household spoon.


Because antibiotics affect the normal bacteria in the body, some people may develop fungal infections, such as oral thrush or vaginal yeast infections, during or after treatment. Persistent diarrhea after completing the antibiotic should also be reported to a healthcare professional because, although uncommon, it may indicate a more serious intestinal infection caused by Clostridioides difficile.


Responsible antibiotic use is essential. Clarithromycin should never be shared with other people or used for illnesses that have not been diagnosed as bacterial infections. Taking antibiotics unnecessarily contributes to antibiotic resistance, which is a growing global public health concern.
